The event is scheduled from 2:00 pm – 3:30 pm EST . The Panel Discussion is planned for 2:15 – 2:30 pm EST (8:15 – 8:30 pm your time). You do not have to stay for the entire event. You may leave after your section.

The event host will provide the 7-10 questions in advance, and you will answer them during the panel interview. The event is scheduled from 7:00 pm – 10:00 pm EST. The Panel Discussion is planned for 8:00 – 8:30 pm. Time subject to change.

Participation in the July 26th in-person event is contingent upon favorable results from this pilot assessment. A positive reception and strong community engagement during the virtual event will support a decision to confirm your role at the in-person event.

Pilot Engagement Assessment

Purpose of Assessment: The virtual event on March 23rd will serve as a pilot to evaluate the suitability of your participation for the in-person event on July 26th. This evaluation will focus on attendance and participant feedback.

Outcome Dependency:

Feedback: Participant feedback must be generally favorable, based on post-event survey.

If these conditions are met, 50% of the fee for the July 26 in-person event will be paid within two weeks following the virtual event.

Contingency for In-Person Event:

If the virtual event receives unfavorable feedback, the agreement for the in-person event on July 26 will become null and void. No additional fees will be paid, and both parties may consider the agreement for the in-person event terminated without any penalties.

Acknowledgment: By checking below, you agree to these terms and understand that your participation and payment for the July 26 in-person event are contingent upon the successful outcome of the March 23 virtual event.

Fee Structure for Event Participation:

In-Person Panel Discussion on July 26:
Fee: $550 USD for a special guest appearance.

Virtual Event on March 23:
Duration less than 30 minutes: Fee $500 USD.

Payment Terms:

Virtual Event:
Payment in full is required 2 weeks prior to the event.

In-Person Event:
Initial Deposit: 50% of the fee is payable within two weeks after the virtual event, contingent on meeting assessment criteria.

Balance: The remaining 50% of the fee is due two weeks before the in-person event.

Pilot Engagement Assessment for In-Person

Event Participation:

The in-person event participation and payment are contingent on the outcomes of the virtual event on March 23. The criteria include:

A minimum of 50 attendees at the virtual event.
Generally favorable feedback from participants.

Outcome Dependency:

If the virtual event meets these conditions, the initial deposit for the in-person event will be processed.
If the conditions are not met, the agreement for the in-person event will become null and void, and no further payments will be due.

Cancellation Policy:

If Optometry Divas cancels any event, Dr. Pelissier will retain any deposits made.
If Dr. Pelissier cancels her participation, any deposits must be returned in full.

Postponement:

Optometry Divas reserves the right to postpone any event. Should this occur:
Dr. Pelissier’s commitment will remain in effect, and Optometry Divas will reschedule based on her availability.
If Dr. Pelissier is no longer available on the rescheduled date, she will retain any deposits paid.

Payment Platform:

Payments will be made through a platform such as PayPal or Wise, based on Dr. Pelissier’s preference, or another agreed-upon platform if she suggests an alternative.

Engagement & Intellectual Property:

I will participate in an interview panel during the Optometry Divas events.
Optometry Divas owns all rights to record, broadcast, and redistribute the content of my interview.

Promotional Use:

Optometry Divas may use my image and recordings for promoting this event and future events without further consent.

Payment Terms:

I will receive a total fee as agreed upon for the panel discussion and the virtual event.
For the virtual event, payment in full is required prior to the event.
For the in-person event, 50% of the fee is contingent upon meeting the criteria from the pilot engagement assessment and will be paid within two weeks following the virtual event. The remaining 50% is due two weeks before the in-person event.
If the pilot engagement assessment criteria are not met, the agreement for the in-person event will become null and void, and no further payments will be made.

Pilot Engagement Assessment:

My participation in the in-person event is contingent upon the success of the virtual event, assessed by minimum attendance and favorable feedback. Failure to meet these criteria will result in the termination of the agreement for the in-person event without further obligation from either party.

Travel and Accommodation:

Optometry Divas will arrange and confirm travel and accommodation, including economy airfare and hotel with a daily allowance for meals and incidentals.

Health and Safety:

Optometry Divas will adhere to all relevant health and safety guidelines to ensure the well-being of all event participants.

Force Majeure:

Neither party is liable for failure to perform its obligations if such failure results from circumstances beyond reasonable control (e.g., natural disasters, pandemics).

Confidentiality:

I agree to keep any proprietary information shared during the event preparations confidential.

Legal and Dispute Resolution:

This agreement is governed by the laws of Florida, and any disputes will be resolved through mediation within this jurisdiction.

This consent constitutes the entire agreement regarding my participation in the Optometry Divas events. I acknowledge that I have read, understood, and agree to these terms.
